Her e-mail address is mhapavilion@yahoo.com, and she can e-mail you a form that must be filled out before the Pavilion will be scheduled or To print a copy of the form directly to apply for scheduling the Pavilion, please click on the button to the left -- and print it from the Adobe Acrobat program. Make certain the "Fit to paper" is checked in the Print Dialog Box. |
